Comprehensive Psychiatric Evaluation: $350
60 minute appointment
The initial psychiatric evaluation is required at the beginning of a patient's treatment. The initial psychiatric evaluation includes a comprehensive assessment of the patient's current symptoms, past medical and mental health history and current life situations
Medication Management/Follow Up: $150
30 minute appointment
Medication management is an essential component of treatment. We work with you to provide the best medication regimen to assist with becoming a healthier you. This includes reviewing current symptoms, effectiveness of medication, medication management, and education. When patients are prescribed medications, they are required to schedule a follow up appointment
Letters for Emotional Support Animals
Forms: 1-2 pages $25
Forms: 3-5 pages $50
We understand that emotional support animals are beneficial to patients with a variety of conditions such as depression, anxiety, and PTSD. If appropriate, we can provide a letter for you so that you are able to have your emotional support animal with you where appropriate.
Bariatric Surgery Clearance: $250
60 minute appointment
We know that weight can have a big impact on one's mental health. We are able to complete a clinical assessment from a psychiatric standpoint. The assessment helps to determine the patient's readiness for surgery, motivation, and all of the mental factors that could cause failure or success of the surgery
We offer additional services at a billed rate of $25 for every 15-minute increment.
These services include, but are not limited to:
- Letter writing
- Accommodation letters/forms
Ultimately it is up to the provider to decide if she is able to provide this service and she is under no obligation to do so.
Cancellation Policy
Your time is valuable and so is ours. We kindly ask that appointments be cancelled at least 24 hours in advance. You will be charged the full appointment fee for all scheduled appointments that are missed or canceled with less than 24 hours' notice. Patients who miss 3 appointments in a row will need to discuss an alternative plan. We are happy to assist in rescheduling your appointment to a more convenient time
Payment Policy
All payments (co-pays, co-insurance, self-pay) are due at the time of service. We accept debit cards and credit cards.
